The main motivation of this study is to propose a new fuzzy risk score for Occupational Safety and Health (OSH) management based on Chang's extent analysis for the pairwise comparisons in fuzzy Analytic Hierarchy Process (AHP). In Bas [1], a Quality Function Deployment (QFD) methodology is employed by using Houses of Quality (HoQs) to determine the relationships between tasks and hazards, and hazards and injury events. Thereafter, the injury events are prioritized by using a risk score. The risk score proposed in Bas [1] for OSH risk assessment is proposed to be fuzzified in this paper instead of using statistical analysis results for the definition of the parameters.
